I've got a list of everything but pasting the excel stuff here doesn't format correctly.  If anyone has any sort of solution to this, or can "host" the excel file somewhere so we can link to it I'll be more than happy to give you guys all the info.

If you can attach the excel file to an e-mail, I'll give it a shot.

I added a link to a PDF file version.  On a dial up line, it will take a good while to load, but it is all in one file and has better formatting.
